Medication Management Canada (MMC) is a leader in health care service delivery logistics. We focus on sustainable and scalable solutions for pharmacists and other health care providers. We have extensive experience including two of the largest scale pharmacy practice change initiatives implemented in Canada.
In 2009, Alberta's Pharmacy Practice Models Initiative (PPMI) engaged close to 200 pharmacists in over 100 pharmacies, delivering patient care services to over 10,000 patients (18,000 encounters). On an even larger scale, the British Columbia Medication Management Project (BCMMP) was made possible by our innovative technology and our proven practice change management approach. Overall, 300 pharmacists and 120 pharmacies were selected to participate in the project, achieving well over 20,000 encounters. Combined, these initiatives have generated over $2M in patient care services revenue for community pharmacies.
We understand what is needed to achieve practice change for pharmacists and pharmacy at the store level. We know that there are many ways to initiate change management and not all of them work. We've learned that pharmacists and pharmacy teams are successful only when they have the right combination of technology, workflow management, performance management coaching, and access to ongoing support resources whenever and wherever services are provided.
